  • Fill in the required fields, then click the Next button

  • The request details page will appear, where you must click the Submit button

  • The user must be an employee in  Health Affairs Program at the Ministry of National Guard

  • The user must have an account in the ERP system used in MNGHA

  • Enter the delegator's information, such as the employee number
