Bilateral (or Unilateral) implantation for sudden hearing loss (previous normal hearing bilateral within the past two years).

Bilateral (or unilateral) implantation of children under 4 years of age.

Unilateral implantation of children under 8 years of age.

Unilateral implantation of individuals 8 to 18 years of age, with hearing loss greater than three years duration that has progressed to severe profound, who are primarily oral/aural communicators .

Unilateral implantation of individuals 8 to 18 years of age, with congenital/ pre-lingual severe to profound hearing loss, who are primarily oral/aural communicators.

Second implant for children under 8 years of age (catch-up category).

Second implant for individuals 8 years of age to 17 years of age, with hearing loss greater than three years duration that has progressed to severe-profound, who are primarily oral/aural communicators.
