Discharge Office
- Deliver a service to enhance the efficiency of the utilization of the Post Discharge Services through our Adult Medical and Surgery Discharge Lounges in coordination between inpatient and outpatient.
Facilitate and coordinate:
- Patients discharged from the hospital within the past 14 days with complex healthcare issues must be addressed immediately.
- Pending services such as a medical report, sick leaves, appointments, etc.
- Patients who are not making the required contact with their primary care physicians shortly after discharge and follow-up care patient needs after leaving the hospital.
