Please submit a request through the Medical Eligibility e-service on the below link:

Please make sure that all data entered is correct.

The sponsor must log in to Patient Services application, then Group Management, and then add the family members and enter the required information.

Please make sure that all data entered is correct for the sponsor and the dependent.

Please make sure that the following data is correct: sponsor date of birth, dependent date of birth, the national identity of the dependent.

The sponsor can electronically open a medical file for first-degree relatives (father, mother, husband, wife, children). For more information, please visit the Registration office.

You will not be able to open a medical file for the son if he is over 18 years old.

You will not be able to open a medical file for the daughter if she is married.
