Home Health Care Scope of Services
Home Health Care Program is a hospital-based, community program, which was established in March 1994 to facilitate discharges from the hospital and for continuation of patient’s care. It was developed to include as goals, not only to decrease hospital length of stay but also to reduce patient’s visits to E.R., re-admission to hospital, visits to outpatient clinics and laboratories and also as an alternative to hospitalization, i.e. Palliative care and Home IV therapy.
The service offers a multi-national and multidisciplinary service to National Guard patients and their families, Services include medical, nursing, respiratory, occupational therapy, clinical nutrition and social work.
