Unadjusted mortality Rate
This is a measure of acute care inpatient mortality from the total number of patients discharged from Hospital.
ER return as level 1 or 2
(ER return as level 1 or 2 or death within 72 hrs of discharge from ER- can we add definition to level 1 and level 2.)
The percentage of patients discharged from the Emergency room and readmitted to Emergency room in a critical
condition
( level one or two) or death within 72 hrs from their first visit.
Readmission Rate with in 30 days
The percentage of patients discharged from the hospital who are readmitted to the hospital within 30 days from the total number of patients discharged.
Harm: Non Harm Ratio
The Ratio between Reported incidents associated with patient’s Harm and Reported incidents with No Harm occurred.
